Social Worker (Post-Acute Care)

Compensation range:

$37.56 - $44.13 - $50.70

Your rate of pay will be based on applicable experience

POSITION SUMMARY:
The Social Worker assists in meeting the psychosocial needs of patients and families. They work with interdisciplinary staff to facilitate effective care management and support to patients. Works in the areas of Homecare, Hospice and Acute Rehab services.

EDUCATION / TRAINING / EXPERIENCE:
Minimum:
- Master’s degree in Social Work (Employees hired into the position prior to March 2017 are only required to have a Bachelor’s degree)

Desired:
- Experience in a medical setting in areas of Oncology, Home Health, Hospice, Acute Rehab or general hospital care

LICENSURE/CERTIFICATION:
- Current CPR certification
- Current California Driver’s License
- Eligible to be insured under the Medical Center’s Liability Policy with DMV Report (and minimum age 21)
- Proof of current auto insurance coverage (no business exclusion)

SKILLS / KNOWLEDGE / ABILITIES:
Must have knowledge of State and Federal funding sources; local community services; psychosocial issues impacting compliance/self-care related to disease or illness; cultural issues; crisis intervention skills, and issues related to special populations. Critical thinking and grammatical skills are necessary to communicate effectively, verbally and in writing. Organizational and multi-tasking skills are essential. Must possess integrity and sensitivity in dealing with complicated patient problems and varied population groups. Ability to maintain confidential information is essential. Must have the ability to document relevant information for the medical record and complete all required documentation. Behavior should be reflective of Enloe Medical Center core values.
