ESSENTIAL DUTIES AND RESPONSIBILITIES
Directs the admission process as well as arranges for transfer of patients to internal system partners and external organizations.
Develops and implements the admissions process in conjunction with leaders across various departments within Care Management.
Determines and implements admissions best practices, revising or refining as indicated; oversees admissions staff.
Develops departmental goals that align with the strategic goals of the organization.
Directly oversees all aspects of admission process for all care management operational locations.
Develops and implements training programs and professional development plans to educate Assessment Managers, Assessment Schedule Coordinators, Assessment Coordinators, and Assessment Nurses on the admissions process.
Participates in team meetings as required.
Responsible for the department meeting established standards of promptness related to admissions and other quality metrics established through regulatory and internal benchmarks.
Facilitates communication with Assessment Coordinators and Care Management offices and staff regarding patient admissions and transitions.
Reviews analytic data to ensure appropriate staffing to meet the needs of each region served by the Department
Facilitates the coordination between Departments to ensure standard of promptness are met and admissions complete to facilitate Level of Care determination.
Maintains communication and relationships with various departments to ensure the assessment process is complete within standards of promptness and that members experience is positive.
Reviews weekly analytic data. Tracks information and develops action plans to address key performance indicators.
Attends and participates in continuing educational programs to keep abreast of changes in their field as well as to maintain current license/certification, as required.
Attends and participates in mandatory in-service trainings.
Honors patient’s rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints.
Complies with Corporate Compliance Program.
Complies with all Privacy & Security programs.
Ensures the Department obtains pertinent information from patients and/or family members, referral sources, appropriate social agencies and medical providers.
Ensures contact and follow up and admission for potential members within regulatory and internal standards of promptness.
May serve as an interim Assessment Manager.
Ensure the coordination between departments to ensure the admissions process is integrated between CM departments.
Promotes the image and reputation of the System by exhibiting servant leadership and providing direct and open lines of communication.
Contributes to the work of committees, workgroups, project management, and other collaborative efforts of the System.
Performs other duties as necessary to ensure the success of the System.
SKILLS AND ABILITIES
Thorough knowledge, adherence and enforcement of current regulations, as well as Care Management Services’ Operational guidelines and best practices related to the operations of the waiver program and Care Management Services.
Maintains constructive working relations by communicating and interacting effectively with supervisors, organizational leadership, peers, internal and external customers, in a positive, professional and respectful manner.

MINIMUM QUALIFICATIONS
Valid RN license with a minimum of 10 years with experience with validated assessment tools, MDS or RAI and at least 7 years in management or supervisory position OR
Bachelor’s degree in Social Work or related field with a minimum of 10 years of in the health and human services field with at least 10 years in management or supervisory position
Certification in Case Management Required
Process Management Experience highly preferred
Valid Driver’s License
Reliable transportation
