The Role:

The Care coordinator is the human anchor of the patient journey. This role is about guiding, supporting, and owning the patient’s healthcare experience - from onboarding through ongoing follow-ups and milestones.

You’re not just coordinating logistics. You help patients make sense of healthcare, stay engaged, and feel continuously supported. With a strong sense of responsibility, you ensure nothing falls through the cracks, and every patient feels seen, guided, and cared for.

You’ll work closely with physicians, Personal Health Managers (PHMs), lifestyle professionals, vendors, and operations teams to turn plans into action.

Your Background:

Experience in a people-facing or coordination role (customer success, patient services, operations, care coordination). Trained and registered nurse background required.

What You’ll Do:

● Guide customers through onboarding and next steps

● Coordinate tests, appointments, and follow-ups

● Maintain ongoing customer communication and engagement

● Monitor progress and flag issues to the care team

● Work closely with doctors, PHMs, operations, and vendors

● Own the customer experience end to end
