Role Description

This is a part-time hybrid role for a Case Manager at Taliaz. The office is located in Petah Tikva, with flexibility for remote work. The Case Manager will work closely with healthcare providers to ensure streamlined care for patients, monitor treatment plans, and facilitate communication between patients and caregivers. Additional responsibilities include data collection and analysis, providing support to practitioners using the Taliaz platform, and maintaining accurate documentation of cases.

Qualifications

• Strong interpersonal, communication, and active listening skills to effectively engage with patients and healthcare practitioners
• Bachelor’s degree in social work, psychology, or a related field is a must
• Ability to organize, document, and maintain accurate patient records and case management files
• Experience in data collection, analysis, and reporting to inform care strategies
• Knowledge of mental health care processes or experience in clinical or healthcare settings is an advantage
• Proficient in using digital tools and technology; ability to quickly adapt to new platforms
• Detail-oriented with proactive problem-solving abilities
• Fluency in Hebrew and proficiency in English and Russian preferred
• Availability to work 10 hours per week
