Hours: Full time, 35hours / week Flexible

Clearbrook is a non-profit, social services agency specializing in providing innovative tools, services and opportunities to individuals with intellectual and developmental disabilities such as Autism and Down Syndrome.

The Coordinator of the Home-Based Services supervises the Self Direction Assistant. The Coordinator reports to the Director of the Home-Based Services and is responsible for the program operations in the absence of the Director. Collaborate with fellow coordinators and other programs within Clearbrook. Establish and maintain contacts with families, guardians and other service providers. Ensure compliance with Clearbrook’s confidentiality policy. Facilitate or attend staff meetings for the effective operation of the program and the proper and timely dissemination of information. Responsible for successfully implementing the PHI coaching approach as well as utilize PHI techniques when communicating with staff. Conduct and attend trainings in accordance with the training policy of the Home-Based Services. Be available by cell phone at all times.

The Coordinator Must Have

• Bachelor's degree in a social service related field
• At least 2 years of experience working directly with individuals with intellectual disabilities
• Valid IL driver's license
• Minimum of 2 years supervisory experience is required
• At least 2 years of case management experience
• QIDP certification
