Position: Nurse Practitioner - PRN/Part Time - Home Risk Assessments

Pay: $90.00 per completed Assessment

Type: PRN/Part Time - Flexible

Hours: Days, Evenings, Nights, Weekends - Flexible Scheduling

Overview

Join our team as a Nurse Practitioner conducting comprehensive health assessments during home visits, skilled facility visits, and telehealth sessions. This unique opportunity allows you to make a significant impact on patient care, emphasizing preventive strategies and closing multiple care gaps with each visit.

Responsibilities

Conduct Adult/Geriatric Assessments, medical history, diagnosis, and treatment

Collaborate with physicians, case managers, social workers, and other medical personnel

Work in various environments including home visits, skilled facilities, and telehealth

Requirements

Master’s Degree or commensurate experience

Current RN and NP licensure in the state of practice

Board certified by AANP, ANCC in an approved specialty

Minimum Of 1 Year Experience As a Nurse Practitioner

BLS, ACLS, or CPR certification

Strong computer skills and familiarity with EMR software

Positive and outgoing personality, flexibility with frequent change

Travel Requirements

Ability to travel with a valid state driver's license and access to a car
