Taliaz is looking for a Mental Health Case Manager – Telehealth Position

Taliaz is revolutionizing the treatment and management of mental health disorders by harnessing artificial intelligence and machine learning. By doing so we enable an easy, effective and rapid patient assessment and an improved prescribing precision and management for a wide range of mental health conditions. We understand that mental health disorders are multifaceted and unique, and through combining data science with machine learning, we translate complex genetic, metabolic, demographic and clinical data into an easy-to-use assessment and prescribing support tool in combination with a full-service offering.

Your Role as a Mental Health Case Manager at Taliaz:

• Prepare and administer initial patient intake through live video sessions to adults suffering from anxiety, depression, and other related conditions.
• Monitor and manage ongoing patient engagement to make sure the treatment plans are being followed, while keeping track of each patient’s progress and ensuring satisfaction.
• Draw on your experience to assist the patient in meeting their goal progress measures, and if need to adjust the treatment plan in consultation with the clinical team.
• Maintain timely clinical documentation and coordinate with the clinical team to make sure proper follow-up and patient management is being deployed.
• Serve an integral role in the evolution of our services and therapy platform, including content and care models by collaborating with our Clinical and Product teams.
• Work with the management to build out a full service therapy team fully focused on various mental health therapies.

What We’re Looking For:

• A Master’s degree in social work.
• Up to date license in the relevant practice.
• Experience providing therapy and interventions for adults with depression and anxiety – An advantage.
• Great interpersonal communication, observant, autonomous and flexible. An open-minded team player with a start-up mindset.
• Continued commitment to learning evidence-based best practices for the treatment of mental health conditions.
• Tech-savviness and enthusiasm for integrating digital solutions into mental health therapy.
• Fluent in Hebrew and English
