Position Title: Patient Care Coordinator
Work Unit: Spine Project
Reports to: Deputy Country Director
Duty Station: Addis Ababa
Start Day: Immediate
Status: Full-time position
Job Summary
The Patient Care Coordinator is responsible for promoting and restoring patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members.
Specific Tasks
Patient Care and Monitoring
- Record patients' medical information and vital signs. Take history of new patients.
- Conduct Pulmonary Function Tests (PFT) and International Normalized Ration checks.
- Accompany patients to clinics & hospitals for investigation and treatment.
- Monitor, record, and report symptoms or changes in patients' conditions.
- Monitor all aspects of patient care, including diet and physical activity.
- Collect results from different laboratories and clinics as well as other medical service providers – CT scan, MRI, etc…)
- Conduct home visits and dress the wounds of post operated patients that are non-ambulatory.
Communication and Coordination
- Communicate with patients and patient’s family to schedule appointments for doctors’ visits and to provide nursing care.
- Prepare patients for and assist with examinations or treatments.
- Schedule patients for appointments and tests and ensure that follow up calls are made in a timely manner.
- Coordinate and facilitate outreach campaigns.
Documentation and Inventory Management
- Prepare surgical files and ensure that all needed x-rays are available. Properly file x-rays, ECHO and other tests.
- Maintain nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies; verifying receipt of supplies; using equipment and supplies as needed to accomplish job results.
Clinical Assistance and Compliance
- Assisting Dr. Hodes at Clinic.
- Protect patients and employees by adhering to policies and protocols, medication administration and storage procedures, and controlled substance regulations.
- Maintain patient confidence and protects operations by keeping information confidential.
- Maintain a cooperative relationship among health care teams by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
- Perform other duties as directed by the supervisor.
Qualifications Required:
- Education: Bachelor’s degree in Nursing or a related field.
- Experience: Minimum of 2 years of nursing experience, preferably in a clinical setting.
- Licenses/Certifications: Valid license to practice medicine in Ethiopia.
Skills:
- Strong communication and interpersonal skills.
- Ability to work effectively in a multidisciplinary team.
- Proficiency in medical software and equipment.
- Strong organizational skills and attention to detail.
- Ability to handle multiple tasks and prioritize effectively.
Salary Range: Negotiable, depending on experience and qualifications.
